These preferences are usually documented and witnessed, and are commonly referred to as “Advance Directives”. Healthcare instructions can be outlined in various documents such as a “Personal Healthcare Directive”, “Appointment of Healthcare Agent”, “Designation of Healthcare Surrogate”, “No CPR Order”, “Organ Donation”, “HIPAA Authorization”, or a “Preneed Guardian Designation”.
